Scenario:
In the care setting you work in, your manager has encouraged you to apply for a senior
post, which requires you to coordinate the care of different individuals who have
experienced, or are experiencing, significant life events. You have been asked to provide
evidence of your practice and ability to apply theoretical principles of providing care and
support to different individuals who have experienced significant life events. The role will
also require you to be responsive, in accordance with the policies and procedures, to the
complex behaviours that are related to coping with change, and the individual differences
that can influence this process.
Part 1: A reflective report of two case studies of individuals you have provided care and
support to:
Your report will be based upon two case studies of individuals you have worked with who
have experienced, or are experiencing, significant life events. The individuals selected must
be of different ages, and at different stages in their lives. You must seek permission to use
details for the case studies and ensure that all information is anonymised including the
setting and location. A reflective report needs to be structured, logical and clearly
presented. It will enable you to review practice and suggest recommendations for improved
practice. The work needs to be suitably referenced and link to evidence-based research. A
bibliography is required using Harvard style referencing.

LO3 Explain how organisational policies and procedures support individuals and their
social networks affected by significant life events
First, you will discuss the impact of expected and unexpected significant life events on
individuals at different ages and stages. Also, the impact of different significant life events
on the individual’s family, friends and wider social networks should be discussed. For a
higher grade, you will critically discuss the relationship between the impact on the
individual, and the impact on their family, friends and wider social networks in relation to
significant life events. In addition, critically evaluate the impact of significant life events on
individuals, their families, friends and wider social networks
In the second section, you will compare how different external voluntary and statutory
agencies support individuals through significant life events. You are also expected

part of a wider team of support services. Critically analyse barriers to partnership
approaches for individual and family affected by significant life events.
In the last section of part 1, you will analyse the relationship between organisational
policies and the practical support available in health and care settings to different
individuals experiencing significant life events. Also, critically review the effectiveness of
policies and procedures in own setting that provide a framework for supporting individuals
experiencing significant life events. Considering legislative frameworks, critically reflect on
organisational approaches to promoting effective and co-ordinated support for supporting
individuals and their social networks affected by significant life events.
To support your reflective report, you must complete the
Reflective Log which is readily
available on VLE. (Appendix – 2)
Part 2: A portfolio of practice-based evidence, including reflective logs
Your placement portfolio will include work product evidence of ways in which you have
demonstrated care, planning and support to individuals who have experienced, or are
experiencing, significant life events. Within the portfolio, your evidence will include witness
testimony, observation, feedback and review documentation, and reflective learning journal
entries. You will provide evidence of ways in which you have worked with others to provide
a coordinated and person-centred approach to caring for, and/or supporting service users.
You are advised to develop a short report to supplement the evidences listed above.
What to do
LO4
Reflect on how individuals who have experienced significant life events are
supported in care service provision
You will develop your practice portfolio by providing evidence of a period of appropriate
support to an individual and their social network who have been affected by a significant
life event. Within the portfolio, provide evidence through reflective learning journals, witness
testimony, feedback and mentor reports, and observation reports. There should be at least
two observations of your practice, carried out by your supervisor on placement who can be
communicated by your tutor / assessor.

Observation report – This will cover the period of your own role in supporting individual
and their social network affected by significant life event. In the observation you will
investigate referral processes available to support an individual and their social networks
affected by significant event. In the same observation, you will
reflect upon the
effectiveness of own role in supporting individuals and their social networks affected by
significant event. (Appendix 1)
Witness testimony – A witness testimony should be completed by the student and at the
end of the attached form write the name of the supervisor who can be contacted for
authenticity. (Appendix 3)
Reflective Journal – reflecting upon your own role in supporting individuals and their
social networks affected by significant life events, as part of the wider network of care
services available to different individuals in your locality. You can use the attached form to
record activities (Appendix -4). You should also provide evidence of your own role in
referring and signposting to other sources of support. You should then produce a
summative report which
critically reflect on how effectively care services in own locality
respond to individuals and their social networks affected by significant life events.

Guide to student
1. Preparation guidelines of the Coursework Document
a. All coursework must be word processed.
b. Any assignments submitted with TEXT BOXES will not be accepted.
c. Document margins must not be more than 2.54 cm (1 inch) or less than 1.9cm (3/4
inch).
d. Font size must be within the range of 10 point to 14 points including the headings and
body text (preferred font size is 11).
e. Standard and commonly used type face, such as Arial and Times New Roman, should
be used.
f. All figures, graphs and tables must be numbered.
g. Material taken from external sources must be properly referred and cited within the
text using
Harvard system
h. Do not use Wikipedia as a reference.
2. Plagiarism and Collusion
Any act of plagiarism or collusion will be seriously dealt with according to the College
regulations. In this context the definitions and scope of plagiarism and collusion are
presented below:

Plagiarism is presenting somebody else’s work as your own. It includes copying
information directly from the Web or books without referencing the material; submitting
joint coursework as an individual effort.
Collusion is copying another student’s coursework; stealing coursework from another
student and submitting it as your own work.
Suspected plagiarism or collusion will be investigated and if found to have occurred will
be dealt with according to the College procedure

3. Submission
a. Initial submission of coursework to the tutors is compulsory in each unit of the course.
b. The student must check their assignments on
ICON VLE with plagiarism software
Turnitin
to make sure the similarity index for their assignment stays within the College
approved level. A student can check the similarity index of their assignment up to
five
times in the Draft Assignment submission point located in the home page of the
ICON VLE.
c. All Final coursework must be submitted to the
Final submission point into the Unit
(
not to the Tutor). The student would be allowed to submit only once and that is
the final submission.
d. Any computer files generated such as program code (software), graphic files that form
part of the coursework must be submitted as an attachment to the assignment with
all documentation.
e. Any portfolio for a Unit must be submitted as an attachment in the assignment
4. Good practice
a. Make backup of your work in different media (hard disk, memory stick, etc.) to avoid
distress due to loss or damage of your original copy.
5. Extension and Late Submission
a. If you need an extension for a valid reason, you must request one using an
Exceptional Extenuating Circumstances (EEC) form available from the Examination
Office and ICON VLE. Please note that the tutors do not have the authority to extend
the coursework deadlines and therefore do not ask them to award a coursework
extension. The completed form must be accompanied by evidence such as a
medical certificate in the event of you being sick, and should be submitted to the
Examination Office.
b. Late submission will be accepted and marked according to the College procedure.
It should be noted that late submission may not be graded for Merit and Distinction.
c. All late coursework must be submitted to the
Late submission point into the unit

Appendix – 3
Witness Statement
A witness statement is used to provide a written record of students’ performance
against targeted assessment criteria. Anyone in the placement who has witnessed
the skills being demonstrated can complete this witness statement. It can be
someone who does not have direct knowledge of the qualification, unit or evidence
requirements as a whole, but who is able to make a professional judgement about
the performance of the student in the given situation.
The quality of a witness statement is greatly improved, and enables the Assessor to
judge the standard and validity of performance against the grading criteria, if:
the witness is provided with clear guidance on the desirable characteristics
required for successful performance
the evidence requirements are present on the witness testimony (this may need
further amplification for a non-assessor)
the student or witness also provides a statement of the context in which the
evidence is set.
The witness statement does not confer an assessment decision. The Assessor must:
consider all the information in the witness statement
note the relevant professional skills of the witness to make a judgement of
performance
review supporting evidence when making an assessment decision
review the statement with the student to enable a greater degree of confidence
in the evidence
be convinced that the evidence presented by the witness statement is valid,
sufficient and authentic.
When a number of witnesses are providing testimonies:
it may be helpful to collect specimen signatures
all witness testimonies should be signed and dated by the witness
information of their job role/relationship with students should also be recorded.
These details add to the validity and authenticity of the testimony and the
statements made in it.
