In person centered care Multi- disciplinary teams are
involved. Their views need to be seen periodically to
ensure all are working as planned or any of the areas
needs attention.

Feedback from family and friends
It is also important to learn from families and friends. How
is everything going? Are the services helping the
individual ?
Family and Friends are most important people in
individual’s life hence their views are important in service
delivery.

Supported decision making and involvement
of individual in planning of services
It is a normal practice in person centered, to involve the
user in every stage of services, i.e., From planning to
delivery.
A person experiencing life changing event must be
involved in decision making and planning of services.

Supervision and monitoring approaches
Ensure a system in place to monitor the progress and satisfaction
of the person in receipt of services.
