Introduction

Care planning is considered as one of the most crucial aspects of the organisation. It is significant in terms of providing proper care the individuals by assisting them with the appropriate treatment. The study will emphasise on the different models of assessment and analyse their implementation in the care setting. Apart from these, the regulatory and legislative frameworks will be analysed to understand their influence in the care setting. The later part of the study will focus on the development of an appropriate care plan that is considered effective in terms of providing holistic care to the individuals and families. Besides this, the paper will look to shed light on the different barriers and challenges faced by the healthcare practitioners while using person-centred care in the organisation. The benefits of person-centred care will be discussed in the course of the study. The care setting that will be selected for completing the study is BMI Healthcare Limited that is located in London (circlehealthgroup.co.uk, 2022).

It is crucial for healthcare companies to focus on developing their care plan with proper implementation of theoretical perspective. The different models will be compared for performing a critical analysis of the same. The influence of theoretical perspective on the development of care planning model will be examined in this section. This will present a clear idea about the care plan used in the care setting.

The performance of the care setting depends on the use of different models. Systems theory, ecological approach and person-centred approach are the three major models necessary for implementing in the functioning of healthcare. The systems theory is focused on enhancing the environment rather than focusing on an individual. It emphasises on the pattern of executing things and aids in the process of gaining the right outcome (Baker, Xiang and Atkinson, 2017). However, the ecological approach emphasises on understanding the relationship between the environment and the individual. This approach focuses on using proper resources in the workplace to support the care workers in the right manner. Besides this, person-centred approach deals with providing individuals to find their own time and use their experience for increasing value. This is used for ensuring that the relationship of the service providers with the service users is meaningful. As a result, it is necessary to understand the implementation of these models for enhancing the performance of care setting.

Besides this, it helps to analyse the different aspects of providing services to their customers. The implementation of this act helps to ensure that the individuals in the workplace are being offered with quality environment, so that they could execute their activities in the desired manner (Sudore et al., 2017). The Care Act 2014 should be implemented to provide personalised care to the consumers. This is significant for developing the overall performance of the care setting. This act aids in the process of dealing with the vulnerable adults who require quality care for their wellbeing. It is crucial to understand that the use of theoretical perspective in the workplace has a major influence on the use of care planning model implied in the workplace. The care-planning model is majorly divided into four phases namely identify, prepare, plan and manage. The first phase on the model helps to identify the challenges and problems persisting in the delivery of care in the workplace. This helps to understand the importance of providing personalised care to the patients to improve their health conditions (Baughman et al., 2017). The care practitioners should prepare a memorandum that allows them to perform their duties in the right way. This is necessary for understanding the exact needs of the patients and helping them in the best way. Besides this, the care practitioners should look to plan their services to suit the needs of the service users. This is effective for retaining customers and providing them with the required services. As a result, the influence of theoretical perspective on the workplace could be analysed easily.

It is crucial to understand the duties and responsibilities of the care practitioners in terms of promoting person centred care to the concerned individual. They are responsible for identifying their health issues and guiding them through the appropriate treatment. This is effective in terms of dealing with the issues of the individuals and providing them with suitable solutions. For understanding this, a case study from The Royal Melbourne Hospital has been used (Scie.org.uk, 2022). The case study shows that the patient named Lola was facing issues in communication with the care practitioner. Besides this, the practitioner did not show much interest in terms of dealing with the patient. As a result, after lodging complaint the care setting was able to get this issue to the wider audience and enhance their services to the other.

Based on my experience, as an intern within the workplace, I have been able to acquire a proper understanding of maintaining quality healthcare for the growth of patient centred care. An individual is able to acquire support and control over the decisions that are made within their life through the patient centred healthcare plans and practices (Nienaber, Yuan and Ernst, 2020). There are different aspects of healthcare that fulfil the action and the healthcare goals that are prescribed by an organization in the development of care planning and growth for the patients. The processes of care planning are used to diagnose the problem and provide suitable recommendations for the growth and development of the wider community. It is relevant to note, that care planning ensures support and growth from the wider community for engaging in activities that are essential, for the development and growth of a healthcare intern. I have learned the importance of building better processes and learning in relation to the determinants of healthcare.

The family members and the carers play an equally important role in the development of holistic healthcare practices. It is beneficial in the growth of wider community through the assessment of the illnesses faced by the patient and in the maintenance of proper standardized and personalised care for the affected patient. Holistic care and planning are imperative determinants for the maintenance of high quality patient care. Support planning is an essential document, for improving the development of the holistic care plan in order to intervene for the purpose of the research.

M5 Importance of team work and leadership in developing supportive approaches for the individual

The significance of teamwork could be considered highly effective in terms of providing supportive approach for the care of an individual. This is effective in terms of working in collaboration with one another and guiding the success factor of the care setting. This is useful for improving the quality of work in the care setting and allows them to deliver the right services to their consumers. This is necessary in terms of developing the overall quality of the treatment being offered to the service users (Salas, Reyes and McDaniel, 2018). The inclusion of teamwork helps in the completion of activities on time and aids in the process of achieving the objectives in the business. Besides this, the role of leadership is considered highly necessary in terms of providing direction to the care practitioners and allowing them to perform their duties in the right way.

The role of the leader is highly crucial in shaping the outcome of the care setting. This is effective in terms of analysing the activities in the workplace. As a result, it is the duty of the leaders to implement the strategies in the workplace for enhancing the performance of the care workers and allowing them to render services to their consumers in a significant way. This is necessary for understanding the objectives in a better way and achieving the same for creating greater values for the consumers and providing them with the right treatment.

The internship helped me to understand the systematic planning process followed by the healthcare setting, for the establishment of the organization (Glajchen,et al., 2022). It implies on the dire research and analysis of the problem faced by the patient in the development of a holistic local document for improving the supportive approaches for the process of the research. My own practice has helped me to comprehend the skills and emotions, which should be practiced for patient management and delivering quality care to the customers. The care planning is essential, for bringing about growth and holistic care for enhancing the needs of the whole community. The services should be essential for the delivery of effective purposes that are vital in meeting with the strategic objectives of the healthcare organization. Upon, reflection, it can be stated that services, necessary for the development for the purpose of the research. The process of healthcare is vital for the development of healthcare setting. Hence, the process of healthcare setting is essential for bringing about self-directed approach for improving the growth of the healthcare setting. Hence, it is vital to note, that various approaches, are necessary for boosting the growth of community for the healthcare setting.

Hence, upon reflection, it can be stated, that healthcare settings, should take into account, the role of processes and support planning for addressing the issues that are faced by the organization. It can be essential for the development and overall outcome of a customer centric healthcare organization.

It is crucial for healthcare companies to focus on developing their care plan with proper implementation of theoretical perspective. The different models will be compared for performing a critical analysis of the same. The influence of theoretical perspective on the development of care planning model will be examined in this section. This will present a clear idea about the care plan used in the care setting.

The performance of the care setting depends on the use of different models. Systems theory, ecological approach and person-centred approach are the three major models necessary for implementing in the functioning of healthcare. The systems theory is focused on enhancing the environment rather than focusing on an individual. It emphasises on the pattern of executing things and aids in the process of gaining the right outcome (Baker, Xiang and Atkinson, 2017). However, the ecological approach emphasises on understanding the relationship between the environment and the individual. This approach focuses on using proper resources in the workplace to support the care workers in the right manner. Besides this, person-centred approach deals with providing individuals to find their own time and use their experience for increasing value. This is used for ensuring that the relationship of the service providers with the service users is meaningful. As a result, it is necessary to understand the implementation of these models for enhancing the performance of care setting.

P2 Application of Theoretical Perspective for Care Planning

It is necessary to understand that companies should focus on using theoretical perspective in their process of care planning that will allow them to provide the best of service to the service users. This is significant in terms of understanding the ways in which the care plan could be developed in a desired manner by implementing it in the workplace for enhanced operations and desired outcomes. The healthcare practitioners should look to understand the ways in which they could communicate with the patients, so that they could be offered with the best treatment in the care setting (Rietjens et al., 2017). The care setting should focus on using process theory and impact theory in the workplace. This is significant in terms of enhancing their growth and development by offering authentic services to their customers. The importance of the impact theory states that the organisation should look to implement changes in the workplace for driving success in terms of achieving their objectives and offering quality care to the service users. Therefore, the application of theoretical perspectives in the workplace could be analysed clearly.

M1 Legislative and Regulatory Framework for Equality and Diversity in Care Planning

It is crucial for the healthcare practitioners to focus on understanding the legislative and regulatory framework that could be implied within the workplace. One of the most important frameworks is Equality Act 2010. This is significant in terms of ensuring that each employee in the care setting in being treated equally. Besides this, it helps to analyse the different aspects of providing services to their customers. The implementation of this act helps to ensure that the individuals in the workplace are being offered with quality environment, so that they could execute their activities in the desired manner (Sudore et al., 2017). The Care Act 2014 should be implemented to provide personalised care to the consumers. This is significant for developing the overall performance of the care setting. This act aids in the process of dealing with the vulnerable adults who require quality care for their wellbeing. As a result, it could be understood that the use of legislative and regulatory framework is considered highly essential for the overall development of the workplace.

D1 Influence of Theoretical Perspective on Care Planning Model used in Workplace

It is crucial to understand that the use of theoretical perspective in the workplace has a major influence on the use of care planning model implied in the workplace. The care-planning model is majorly divided into four phases namely identify, prepare, plan and manage. The first phase on the model helps to identify the challenges and problems persisting in the delivery of care in the workplace. This helps to understand the importance of providing personalised care to the patients to improve their health conditions (Baughman et al., 2017). The care practitioners should prepare a memorandum that allows them to perform their duties in the right way. This is necessary for understanding the exact needs of the patients and helping them in the best way. Besides this, the care practitioners should look to plan their services to suit the needs of the service users. This is effective for retaining customers and providing them with the required services. As a result, the influence of theoretical perspective on the workplace could be analysed easily.

From the above discussions, it could be understood that the use of theoretical framework is done inside the care setting for improving the business outcomes. This helped to understand the importance of some legislative and regulatory framework that was needed for the business. Apart from this, the care planning in the care setting has been explained for providing quality care to their service users.

Case Study

P3 Responsibilities and duties of own role in promoting person centred care planning in reference to the individuals

It is crucial to understand the duties and responsibilities of the care practitioners in terms of promoting person centred care to the concerned individual. They are responsible for identifying their health issues and guiding them through the appropriate treatment. This is effective in terms of dealing with the issues of the individuals and providing them with suitable solutions. For understanding this, a case study from The Royal Melbourne Hospital has been used (Scie.org.uk, 2022). The case study shows that the patient named Lola was facing issues in communication with the care practitioner. Besides this, the practitioner did not show much interest in terms of dealing with the patient. As a result, after lodging complaint the care setting was able to get this issue to the wider audience and enhance their services to the other.

It is crucial for the higher officials of the care setting to make sure that they are aware of the different risks that might develop in the workplace. They should look to perform a proper risk assessment that will allow them to analyse their business efficacy and aim to reduce the risk factors in the care setting. There are times, when the vulnerable adults are at health risks and they cannot perform their activities without guidance (Sudore et al., 2017). However, the absence of the care practitioner could be a major risk for the individual in the personalised care. The managers should look to ensure that they are keeping the employees engaged in terms of delivering quality services and reducing the major risk factors that could lead to challenges.

D2 Challenges of developing care plans that meet the needs of individuals

It is observed that the healthcare practitioners face numerous challenges in terms of developing the care plan for the needs of the individual. It is seen that the care workers are unable to deal with the demands of the service users. As a result, it is crucial to develop a care plan that will allow them to provide personalised care for the individuals using their service (Moore et al., 2017). The higher officials often face employee resistance while following the care plan. As a result, it is necessary for the management of care setting to provide the workers with proper training and development, so that they become competent enough to use the care plan for delivering quality services to the patients.

In some cases, it is witnessed that the care practitioners and higher officials face barriers in terms of implementing their care plan. Due to this, it becomes difficult for the care setting to deliver the services to the customers. Besides this, the care practitioners often face issues in terms of following the requirements of the care plan. This tends to affect the overall productivity of the organisation and reduces their generation of revenue in the market (Britten et al., 2017). The resistance of the employees play a major role in deteriorating the implementation of the care plan in the workplace. The management authorities look to make sure that the plan is designed in a way to ease the services to the customers. This is necessary for improving the overall outcome of the care setting.

This is effective for improving the overall performance of the workplace and gaining quality outcomes in the operation. This is significant in terms of implementing proper care plan according to the needs of the patients. With the help of this, it becomes easier for the management of care setting to develop the level of communication and guiding the workers for providing quality treatment.

It is vital to encompass an understanding of the values and systems that are essential for the purpose of the care process. It is relevant for improving the lifestyle and the health related factors for the purpose the business. It influences the proper understanding of the conditions that impact control of the business (Levitt, 2021). Hence, the role of the individual, is to provide patient centric for the process of developing strategies for the growth of individual. It is used to cater to long term conditions and enhance the overall strategic operations for the purpose of the business in healthcare.

The aspect of care planning is subjected to challenges and barriers, for developing a patient centric holistic approach (Offer, 2022). The care planning strategies can be vital for boosting growth and outcomes that are development of healthcare setting. There may be barriers in the implementation of social care staff, for enhancing the opportunities of legal and ethical factors, for healthcare setting. Cognitive and emotional barriers and unprepared healthcare issues can affect the purpose of the healthcare systems. These barriers can be overcome with the use of proper patient centred approach for the purpose of healthcare setting. Staff satisfaction and catering to the quality of the individual for the purpose of the healthcare, is a quality to overcome for the healthcare setting. It is essential, as a are practitioner, to be aware of the challenges, and the areas, that affect and influence the overall priorities of quality healthcare systems. Hence, it is relevant to note, that there are differences in maintenance of healthcare planning and growth for the operation of the healthcare setting (Frielitz,et al., 2021).

P6 Benefits of the use of care plans in ensuring the needs of the individual are prioritised

There are several benefits, which can be acquired via the healthcare for the resolution of the issues faced by the healthcare setting. It is essential, for the organization to comprehend the nature of personalised healthcare services. It can be essential, for the development of outcomes and opportunities that are beneficial for the purpose of holistic healthcare setting. It is vital to comprehend the qualities of healthcare and to improve the optimal responses and statuses of the individuals, in terms of ensuring and effective and proper healthcare and planning intervention for the maintenance of healthcare setting. Several benefits can be acquired through the development of intervention plans that are essential, for growth of outcomes that are associated with the development of quality healthcare for the organization. The benefits of care plans are vital for empowering an understanding on the importance of analysing and diagnosing issues faced by the patients. It is vital in building and developing holistic values for the development of business.

P8 Aspects of the care planning process in terms of its impact on the individual

Based on my experience, as an intern within the workplace, I have been able to acquire a proper understanding of maintaining quality healthcare for the growth of patient centred care. An individual is able to acquire support and control over the decisions that are made within their life through the patient centred healthcare plans and practices (Nienaber, Yuan and Ernst, 2020). There are different aspects of healthcare that fulfil the action and the healthcare goals that are prescribed by an organization in the development of care planning and growth for the patients. The processes of care planning are used to diagnose the problem and provide suitable recommendations for the growth and development of the wider community. It is relevant to note, that care planning ensures support and growth from the wider community for engaging in activities that are essential, for the development and growth of a healthcare intern. I have learned the importance of building better processes and learning in relation to the determinants of healthcare.

The family members and the carers play an equally important role in the development of holistic healthcare practices. It is beneficial in the growth of wider community through the assessment of the illnesses faced by the patient and in the maintenance of proper standardized and personalised care for the affected patient. Holistic care and planning are imperative determinants for the maintenance of high quality patient care. Support planning is an essential document, for improving the development of the holistic care plan in order to intervene for the purpose of the research.

M5 Importance of team work and leadership in developing supportive approaches for the individual

The significance of teamwork could be considered highly effective in terms of providing supportive approach for the care of an individual. This is effective in terms of working in collaboration with one another and guiding the success factor of the care setting. This is useful for improving the quality of work in the care setting and allows them to deliver the right services to their consumers. This is necessary in terms of developing the overall quality of the treatment being offered to the service users (Salas, Reyes and McDaniel, 2018). The inclusion of teamwork helps in the completion of activities on time and aids in the process of achieving the objectives in the business. Besides this, the role of leadership is considered highly necessary in terms of providing direction to the care practitioners and allowing them to perform their duties in the right way.

The role of the leader is highly crucial in shaping the outcome of the care setting. This is effective in terms of analysing the activities in the workplace. Besides this, the leader looks to identify the major issues and challenges that have a tendency to affect the wellbeing of the care settings (Lacerenza et al., 2018). As a result, it is the duty of the leaders to implement the strategies in the workplace for enhancing the performance of the care workers and allowing them to render services to their consumers in a significant way. This is necessary for understanding the objectives in a better way and achieving the same for creating greater values for the consumers and providing them with the right treatment.

D3 Reflect upon the planning process in relation to own practice in the healthcare setting

The internship helped me to understand the systematic planning process followed by the healthcare setting, for the establishment of the organization (Glajchen,et al., 2022). It implies on the dire research and analysis of the problem faced by the patient in the development of a holistic local document for improving the supportive approaches for the process of the research. My own practice has helped me to comprehend the skills and emotions, which should be practiced for patient management and delivering quality care to the customers. The care planning is essential, for bringing about growth and holistic care for enhancing the needs of the whole community. The services should be essential for the delivery of effective purposes that are vital in meeting with the strategic objectives of the healthcare organization. Upon, reflection, it can be stated that services, necessary for the development for the purpose of the research. The process of healthcare is vital for the development of healthcare setting. Hence, the process of healthcare setting is essential for bringing about self-directed approach for improving the growth of the healthcare setting. Hence, it is vital to note, that various approaches, are necessary for boosting the growth of community for the healthcare setting.

Hence, upon reflection, it can be stated, that healthcare settings, should take into account, the role of processes and support planning for addressing the issues that are faced by the organization. It can be essential for the development and overall outcome of a customer centric healthcare organization.

Conclusion

From the aforementioned discussions, it could be concluded that the development of care plan is highly necessary for the wellbeing of the patients and the care workers. This helps to understand the use of theoretical framework in terms of shaping the outcomes of the care setting. The study focuses on the implementation of legislative and regulatory framework in the care setting in terms of ensuring that the care workers and the patients do not feel discriminated in the workplace. This is effective in terms of creating enhanced opportunities for the care workers and guiding them to execute their duties to the best of their abilities. The study helps to understand the ways in which leadership and teamwork could be used for improving the quality of care for the patients and enhancing their experience in the workplace. This is necessary for understanding the different issues for implementing the care plan in the right way.
